Notice of Data Breach

Summit Reinsurance Services, Inc. (“Summit”) has notified Select Health Network, Inc. (“Select”) of a data incident that occurred at Summit. Summit is a company that provides reinsurance services to insurance companies and worked with Select in 2007. Select’s systems were not affected by this incident.

On August 8, 2016 Summit discovered that ransomware had infected one of their servers containing certain personal information in April of 2016. Names, identification numbers (which contained social security numbers) and some personal health information and charges of patients of Advantage Health Solutions and M-Plan were on the affected server. The Patient Personal information was from a period during 2005 - 2007.

To help protect your identity, we recommend you take immediate steps to protect yourself from further potential harm. This may include the following precautions:

Select has posted this notice for patients who had Advantage Health Solutions (“Advantage”) or M-Plan insurance utilizing the Select Health Network between 2005 and 2007. Up to 119 former Advantage and M-Plan patients with coverage during this time period may be affected by the Summit data security event, which may affect the security of their personal information. Select has provided Summit with the most recent addresses that are on file so that these patients can be contacted directly by Summit. Due to the age of the data affected, the most recent address on file may not be accurate.
