Q. How are participants identified for the Select Your Health program?
Select Your Health nurses contact individuals who may have multiple medical conditions or may be at higher risk for future medical problems, and work to help improve their health. Individuals are contacted by telephone, and the program and benefits are described to prospective participants. The Select Your Health program is voluntary, and participants may begin and terminate their participation at any time.
Q. What will Select Your Health do for participants?
Program nurses offer case management services including education, follow-up, links to services and information, facilitation, and coordination of services. Emphasis is placed on personal accountability for health and reduction of unplanned/emergency utilization of services.
Q. How does Select Your Health work with physicians?
Select Your Health nurses work collaboratively with physicians to help carry out their treatment plan. Program nurses speak with physicians and/or their staff periodically to provide updates and discuss progress. Through active participation in the program, members frequently experience fewer emergency situations between regular office visits.
Q. How does Select Your Health interact with employers?
Select Your Health staff are available to meet with employers and provide summary data related to outcomes and cost savings for their insured population.
Member-specific information is not shared with employers.
Q. What kinds of outcomes are expected?
Improved health and wellness usually results in decreased utilization of health care services, especially emergency and urgent services. Employees frequently report increased perceived wellness, increased satisfaction and decreased missed workdays.
