How to Enroll in Select Your Health - Health Enhancement

HMO Members

Advantage Health Solutions HMO members have the opportunity to enroll in Select Your Health in several ways:

Select Your Health Nurse Contact

Select's predictive modeling software assists a Select Your Health nurse in identifying individuals who may have multiple medical conditions or may be at higher risk for future medical problems.

These individuals are contacted by telephone by a Select Your Health nurse who will describe the program and its benefits. Select Your Health is voluntary, and participants may begin and terminate their participation at any time.

Physician Referral

Prospective participants may be referred by their physician to join Select Your Health. Our nurses work collaboratively with physicians to help carry out their treatment plan.

Member Self-Referral

Participants may self-refer to join Select Your Health. To begin working together toward common goals, Select Your Health nurses contact the member's physician to establish an appropriate treatment plan.
