Select Your Health is a health enhancement program that works with members to help improve their overall health and wellbeing. Select Your Health Registered Nurses work collaboratively with participants and participants' physicians to develop individual treatment plans and provide the education and support to achieve those goals.

Select Your Health starts by establishing relationships with willing enrollees and works with them to improve their health outcomes through intensive case management, education, integration and coordination of health care services.

Select Your Health members have one or more chronic conditions, such as diabetes, hypertension, elevated cholesterol, arthritis, and many other medical conditions. Some participants also have goals related to weight management. Because Select Your Health focuses on the person and not a disease or condition, the care is individualized to the specific needs of each participant. Health and wellness are promoted through education, follow-up, and links to community services.

Select Your Health nurses help to carry out the members' treatment plan and confer with physicians and/or their staff periodically to provide updates and discuss health status. Through intensive case management, members should experience fewer emergency situations between regular office visits.
